Asphalt Specialists, Inc. Health Insurance Program
Each full time employee employed by Asphalt Specialists, Inc. for ninety (90) consecutive days is eligible to participate in the Company’s Health Insurance Plan effective the 1st day of the month or the 1st enrollment opportunity following said ninety (90) day period, whichever applies.
Asphalt Specialists, Inc. provides a comprehensive Health Care Plan program for regular full time employees; subject to the plan’s enrollment requirement and procedures. This plan includes Medical, Dental, Vision, and Life Insurance options from which to choose. These health care insurance options are intended to provide employees and their families a broad range of protection to meet their health care needs.
The premium amount incurred by both Asphalt Specialists, Inc. and the employee will vary depending upon eligibility requirements and/or the options selected, and whether or not the employee elects to participate in the Health Insurance Plan(s).
If an employee does not meet the eligibility requirements or elects not to participate in the Health Insurance Plan(s), the employee will be wholly responsible for obtaining and paying for his or her own health insurance. If at a later date an employee decides he or she wants to participate in the company’s Health Insurance Plan(s), then said employee will only be able to enroll during open enrollment.
Once a year during open enrollment, employees will have the opportunity to make changes to their insurance coverage. The insurance benefit year begins June 1st and ends on May 31st.
Employees experiencing a qualifying event (marriage, death, divorce, birth, adoption) may make changes to their insurance at the time of the qualifying event, but must do so within 30 days.
Some restrictions and benefit maximums may apply. Be sure to carefully review the specifics of this program in the plan booklet which is available from the Human Resources Department.
Should an employee decide to opt out of our medical coverage and can annually provide proof of their own medical coverage, then he or she is eligible to receive $150 per month from Asphalt Specialists, Inc. provided that said employee has worked a minimum of 30 hours a week with at least 120 hours worked the month to receive the bonus waiver the following month.
The Health Insurance Plan is a company benefit Asphalt Specialists, Inc. provides at its discretion. Asphalt Specialists, Inc. reserves the right to change insurers, cancel any policy, or restrict a policy in any way it deems appropriate, without having to justify a basis.
Upon employee termination, insurance coverage will terminate the last day of the month. Notification will be sent to the COBRA company at that time.
The Consolidated Omnibus Reconciliation Act (COBRA) gives workers and their families who lose their health benefits the right to choose to continue group health benefits provided by their group health plan for limited periods of time under certain circumstances such as voluntary or involuntary job loss, reduction in hours worked, transition between jobs, death, divorce, and other life events. Qualified individuals may be required to pay the entire premium for coverage up to 102 percent of the cost of the plan.
COBRA generally requires that group health plans sponsored by employers with 20 or more employees in the prior year offer employees and their families the opportunity for a temporary extension of health coverage (called continuation coverage) in certain instances where coverage under the plan would otherwise end.
COBRA outlines how employees and family members may elect continuation coverage. It also requires employers and plans to provide notice. |
